How do I fill in the Warrant of Possession section 4 details?

Hi, I’m really confused about what to put in the warrant detail section 4. What amount I should put in (a) and (b)?

Or shall I leave it blank as the bailiff is not there to collect money but only to take the tenant out?

Shall I put £130 in the issue fee? As it’s the cost for applying the warrant of possession?

There is no instruction goes with this form on the Government website and the info online I found is quite mixing. It really confused me.
